The St. Charles Public Library’s Outreach Services Team delivers materials to participating senior facilities, nursing homes, and senior residential complexes, as well as arranges service for individuals living in their homes, due to low vision and mobility challenges.

I acknowledge the following information for this service:
• Materials will be delivered to the patron by Outreach Services staff on a prearranged and mutually agreeable schedule.
• Outreach Services will coordinate the delivery method based on the patron’s preferences.
• Books, CDs, audiobooks, and most other materials have an 8-week checkout period. New materials including DVD’s have a 4-week checkout period.
• Outreach accounts are handled by Library staff. I agree to allow Outreach staff to receive all communications concerning my account.
• I understand my account is fine-free and intended for my use only.
• Materials generally are to be returned at the next visit. If more time is needed, inform the Outreach staff.
• Notify Outreach staff if Home Delivery service is no longer needed.
• Lost and damaged items are the responsibility of the patron or facility depending upon prior contractual agreements.
• Outreach Services evaluates fees for lost and damaged items on a case-by-case basis.
